Hallucinogen persisting perception disorder: A literature review and three case reports

hppd disorder

Researchers have found that certain medications have been useful at helping improve HPPD symptoms in some cases. However, because of the small study sample sizes, the evidence for the use of these drugs isn’t very strong. Furthermore, these drugs come with their own risks and, in some cases, can worsen HPPD symptoms or a person’s quality of life. Because there is little research into HPPD, it’s unclear how common it is.

۲٫ Substances That Induce HPPD

hppd disorder

The visual disturbances experienced, are understood as pseudo-hallucinations rather than the clinical view of hallucinations, as people with HPPD understand what they are experiencing. Sufferers know that their perceptions are a result of alterations in visual processing, as opposed to other mental illnesses whereby the sufferers are unable to discern what reality is. Previous use of hallucinogens is necessary, but not sufficient, for diagnosis of HPPD, and the symptoms cannot be due to another medical condition.

Health Conditions

Scientific sources suggest that HPPD may affect more than 50% of hallucinogen users and this disorder is often underdiagnosed. Therefore, patients suffering from HPPD can present in various clinical settings, and clinicians should be aware of this condition. Nevertheless, further studies are set to continue investigating psychedelic treatments, and advocates hope that further research will accomplish approvals of therapeutic psychedelic substances in the future.

۳٫ Clinical Features

hppd disorder

More research is needed to understand the changes Oxford House in the brain that cause HPPD symptoms. However, in HPPD, they tend to be less intense — it is mostly vision that is affected, rather than the other senses. It is usually apparent to a person experiencing HPPD that they are not seeing things in the way they used to. Professional Reference articles are designed for health professionals to use. They are written by UK doctors and based on research evidence, UK and European Guidelines. If HPPD affects performance at work or school, people with the condition can seek accommodations.

  • There are also reports of people who have only used hallucinogens once or twice experiencing similar symptoms.
  • Often the patient was unable to focus properly with her eyes and tired rapidly while performing intense visual tasks – these deficiencies being detrimental to her studies and professional work as an architect.
  • Observational studies and case reports evaluating clinical presentation.
  • Instead, episodes happen suddenly, without a sign that they’re coming.

Regardless, every perceptual symptom that was experienced during intoxication may re-occur following hallucinogen withdrawal. We report a list of the main literature-reported visual disturbances in Table 2. Throughout the years HPPD and the term “flashbacks” has been used interchangeably, leading to much confusion, lumping together an extremely serious condition, with that of a benign and temporary occurrence. Flashbacks are a transient and intermittent re-experience of perceptual changes reminiscent of a previous psychedelic state. These occurrences do not happen often, do not last long, and do not produce anxiety or unease in the experience. HPPD conversely, is the continuous and often long term presence of disturbances in visual and sensory perceptions.

  • Hallucinogen Persisting Perception Disorder (HPPD) is a cognitive disorder in which individuals continuously re-experience visual and other sensory hallucinations that they first experienced while intoxicated.
  • In addition to medication, mental health professionals frequently recommend talk therapy as a treatment for HPPD.
  • The exact pathophysiologic mechanism underlying HPPD is poorly understood.
  • When the person with HPPD experiences a flashback, loved ones can help them feel safe and remind them that symptoms are temporary.
  • These selective serotonin reuptake inhibitors (SSRIs) alleviated depression but did not relieve the HPPD symptoms.

What Are the Symptoms of HPPD?

  • Surprisingly, both other cases belonging to HPDD type II appeared in patients who consumed ecstasy, although MDMA is generally not considered a hallucinogen and hallucinations are not frequently reported after MDMA consumption.
  • For this reason, it’s important you work with a doctor to find a treatment technique or coping mechanisms that help you handle the disturbances and feel in control when they do occur.
  • Lauterbach et al. reported the unique case of HPPD induced by the atypical antipsychotic Risperidone 60.
  • However, in HPPD, they tend to be less intense — it is mostly vision that is affected, rather than the other senses.

Rather, a multifactorial origin of HPPD-related phenomena is to be assumed that may differ from case to case. The range of case-specific variables may extend from learning and kindling effects, individual reaction patterns to mental trauma and weak self esteem to other psychophysic vulnerabilities Hermle et al. 2008. Additionally, only a small spectrum of hallucinogens seem capable of eliciting flashbacks, with LSD being the leading causative agent. Closely related to LSD in its psychotropic actions is psilocybin, which produces similar but shorter-lasting intoxications. Interestingly, hppd symptoms there is only one documented case of HPPD following ingestion of Psilocybe semilanceata mushrooms in the psychiatric literature, despite its common use in the hippie subculture of the 1960s and 1970s Espiard et al. 2005. The incidence of mental disorders in 200 Native Americans of the Navajo tribe after ritual use of mescaline was the subject of a recent study by Halpern Halpern, 2003.

Currently, most psychedelic drugs are federally illegal, except for certain ketamine products. Therapeutic treatments using psilocybin are legal in Oregon and Colorado, and other US states have made attempts at decriminalizing or legalizing the substance. The only country where therapies using MDMA and psilocybin are legal is Australia. A repeat assessment 19 months later showed a continued improvement of her already normal to above average attention performance which was at odds with the patient’s subjective perception of diminished concentration faculties in everyday life. No change was observed in her mental state, with a persistent low to medium depression indicative of a dysthymic disorder (Beck Depression Inventory IV, Self-report Symptom Inventory 90 Items – Revised).
